A single gateway to a range of services designed to help people over the age of 65 remain in their own homes supported by community services.
Who is the service for?
Signpost is available to anyone over 65 living in a property that has a Council Tax liability to Shropshire Council.
How does it work?
When a Partner agency is in contact with a service user they complete a simple Signpost form. The form collects basic contact details and then asks about a dozen simple yes/no questions. The form is then sent to the Signpost Co-ordinator who acts as a ‘clearing house’ and makes the appropriate referrals to other Partner agencies.
What are the benefits of Signpost?
It enables Partner agencies to access a wide range of services for people they meet and in a simple efficient manner, saving time and resources. For the service user, ‘Signpost’ ensures that he/she receives a range of services which are available to them through a seamless process.
Who Signposts?
At present, the Signpost assessment form is completed done by “professional” people who have been trained in completing the form. “Professional” is used here to describe any person working for or with Partner agencies.
Who are the Partners?
Shropshire Council, Community Council of Shropshire, Shropshire Fire & Rescue Service, Primary Care (your GP surgery), Joint Benefits Team, West Mercia Constabulary and Voluntary organisations.
What services are available?
· Home fire safety checks (fitting smoke detectors, giving advice & information on fire safety & means of escape issues);
· Benefit advice (this includes assisting people to claim all the state benefits and allowances to which they are entitled by helping to complete claim forms, challenging decisions that do not appear to result in appropriate awards of benefit). Help with completing Blue Badge applications for people with mobility problems;
· Grants, energy efficiency advice and practical assistance to overcome fuel poverty/heating difficulties;
· Social Care services where people meet local eligibility criteria;
· Home security/crime prevention advice and assistance, including door and window locks, security chains, external security lights, peep-holes etc;
· Risk assessments and advice to minimise the dangers from falls;
· Community meals services, both hot and frozen meals are delivered to people’s homes;
· Advice, information and support for Carers;
· Information & advice on local social and physical activity providers.
